Create a lead capture form

Create a form template, place it on a page, and review captured contacts in Leads. A template defines the form's design; a placement defines where it appears and who sees it.

Before you start

Your site needs the Forms module and a zone where the form can appear. Decide which contact fields and consent language to use. Check Publishing: with Review before publishing off, a saved placement becomes eligible for delivery immediately.

Create and place a form

  1. Open Forms and click New Form.
  2. Set the internal template label, Header title, Submit button text, and Included fields. Choose the colors and form width, then review the consent statement and preview.
  3. Click Create template.
  4. Open Placements and choose Add placement. Select your saved design under Start from template.
  5. Choose the page section, insertion position, and audience. Select All visitors (everyone) explicitly when the form should have no narrower segment target.
  6. Review the placement and click Place form. Reopen it to inspect its On page preview and adjust the saved design.
  7. If Review before publishing is on, open this zone in Content, choose Publish changes, review the form variant, and click Publish now.

Check capture and drop-off

A placement copies the template's design. Later template edits do not update existing placements; edit each placement that needs the change.

Open Leads to review captured contact records. The Funnel tab shows form views, starts, submits, and abandons. Open a form for field-level details. Form activity and a captured contact record are different records, so check both when investigating a missing lead.

If a form is missing, check its zone, page match, audience, published state, and site launch mode. If contact capture is disabled, review Passive Pilot Mode on Consent. If Forms is unavailable, ask an admin to check the site's module settings.
